Residences at Vantage Point is proud to announce that our community has earned the Health & Wellness Gold Award in the LCS 2025 Health & Wellness Navigation Program Community Awards. In fact, this year’s recognition marks a significant milestone, as our community advanced from Bronze status in 2024 to Gold in 2025.
Commitment Recognized with the Health & Wellness Gold Award
The Health & Wellness Gold Award honors communities that help residents achieve “A Better Life Through Better Living.” Through this initiative, our Health & Wellness Navigation Program partners with residents and families to create personalized care plans that promote health, independence and well-being.
During 2024–2025, our Navigation team provided 1,720 support services, including wellness checks, case management and transitional care after medical events. Because of this commitment, residents benefit from comprehensive, coordinated support that enhances daily living and long-term wellness.
Collaboration and Connection
At Residences at Vantage Point, the Health & Wellness Gold Award reflects the dedication of our interdisciplinary team. Sunnie Sun, LMSW, Independent Living Social Worker, and Odelia Alornyeku, RN, Wellness Center Manager, lead the program together, blending social work and clinical care to meet residents’ individual needs.
“Our work is strengthened by being part of the broader LCS Navigator network,” said Sunnie Sun. “Across all LCS communities, Navigators meet monthly to share ideas and discuss challenges. That collaboration expands what we can offer our residents because we’re constantly learning from each other.”
Sunnie added, “Odelia focuses on the medical side, following up when residents return from the hospital or a procedure. She performs wellness checks and coordinates home care when needed. Residents appreciate knowing that someone will be there to check in and help them recover safely.”
Supporting Residents at Every Step
The Health & Wellness Gold Award also recognizes our commitment to aging in place. Our Navigation team provides strategies that help residents remain independent while offering support through transitional care when needed.
Collaboration is key. Our team works closely with internal departments such as dining, community life and housekeeping, while also coordinating with external healthcare providers and home health services. As a result, residents receive personalized, proactive care designed to adapt to their changing needs.

Earning the Health & Wellness Gold Award is a community-wide accomplishment that reflects our shared dedication to resident well-being. At Residences at Vantage Point, we remain committed to creating an environment where every resident can thrive—physically, emotionally and socially.
